Client Rights & Responsibilities

We support and care for our clients, their families, and the community with compassion, honesty, dedication, respect, and responsibility.

We will:

  • Treat you with respect and courtesy
  • Keep your personal health records confidential
  • Give you time to talk
  • Listen to your concerns
  • Support you as you make important decisions about your well-being
  • Talk to you about getting a second opinion, if required
  • Explain what will happen if you consent to treatment or refuse treatment
  • Provide you with the best primary health care possible

You, as our client, have some responsibilities as well.

You are expected to:

  • Be on time for appointments
  • Treat our staff and others at the CHC with respect and courtesy
  • Tell us about your health and well-being
  • Let us know when your personal information has changed
  • Ask us questions

Client Privacy

CSC CHIGAMIK CHC is responsible for the information it holds regarding clients. Staff at CSC CHIGAMIK CHC must respect and work within organizational policies regarding confidentiality and privacy, consent, and release of information as well as the Personal Health Information and Privacy Act, and the standards of their college.

CSC CHIGAMIK CHC is committed to providing accessible, culture-based, holistic primary care and allied health services and programs. CSC CHIGAMIK CHC​ is an inclusive place where all community members are welcome under “one roof”. Accountability Reports

The centre de santé communautaire Chigamik Community Health Centre is a non-profit, community-governed, primary health care organization located in Midland, Ontario. This unique centre serves the Indigenous and Francophone communities of the entire North Simcoe and Muskoka LHIN and the Anglophone community of Midland, Penetanguishene, Tay, and Tiny. Chigamik provides a combination of primary care, allied health services, health promotion programs, traditional Indigenous medicine, and culturally relevant community development initiatives which are holistic, and offered in French and English.

The following reports provide our clients and stakeholders with access to quality and accountability information and resources.
